Fees
$125 flat fee for initial home visits and $100 for follow-up visits
$20 per additional sibling (twin, triplet)
$20 additional mileage fee applied for services offered more than 20 miles from our office location in West Linn
*If needed, we can provide for your purchase some breastfeeding assisting tools/supplies. These will incur additional cost to the visit fee.
Payment is due at the time of service.
Insurance Information
The Affordable Care Act of 2012 requires coverage of lactation consulting as a “Women’s Preventive Service.” Please contact your insurance company for your coverage details.
At this time we cannot bill insurance directly, but we will accept payment on the day of the consult and provide you with the receipt necessary for you to seek insurance reimbursement. We are not able to tell exactly what any particular insurance policy or plan will or will not cover. A link below will take you to information from The National Women’s Law Center which may help you to understand how to obtain these new breastfeeding health care benefits.
Instructions on how to file for Insurance reimbursement
- Please call your insurance company and ask if they have any particular instructions you need to know or forms that they require.
- After the consult, you will receive a receipt – known as a superbill – which includes:
- A National Provider Identification (NPI) number and tax ID (EIN) number.
- A code for the type of appointment (CPT code), such as a home visit.
- A code for the problem (ICD-9 code), such as slow weight gain in newborn.
- The amount you paid.
- The date of service.
- The IBCLC’s contact info and signature.
- Mail or fax this superbill, along with your policy information, to your insurance company, according to their guidelines or recommendations.
